Required Documents for Y.O.U. Summer Jobs
If I am selected to have my eligibility determined, what documents will I need to participate in the program?
If the teenager’s application is selected, the parent/guardian will have to attend a eligibility determination appointment. The parent/guardian must have the following documents at this appointment:
Proof of Family income
If Parent/Guardian works: 2 most recent pay stubs* or W2 from 2023 or IRS Tax Transcript from 2023
If you receive: Worker’s compensation or unemployment benefits, you will need the award letter
If you do not have any income: You must show proof of how you support yourself and your family (cash assistance, Social Security benefits, SNAP food assistance, etc.).
Proof of Identity and Residence for the parent / guardian
Your current valid State Issued photo ID, Driver’s License, or Passport.
Social Security Card
Proof of current residency in Cuyahoga County*
If your current residence is not listed on your Photo ID, you must provide:
Utility bill, government agency letter that is less than 60 days old at time of your appointment, or a lease agreement.
Documents required for the teen (who will be working in the program) – ALL following documents are required:
Your child’s current photo ID
14 – 17 years old: School ID, School Report Card with Photo, Driver’s License, State ID, Passport, or City Rec ID
18-19 years old: current valid state issued ID: Driver’s License, State ID, or Passport
Proof of Relationship between you and child
Examples of Proof of Relationship:
Birth Certificate of child that has you listed as parent
Court Custody Document showing guardianship
School Record showing guardianship
Medical Record showing guardianship
Government Letter listing household members
Your child’s Social Security CARD (letters WILL NOT BE ACCEPTED).
If child is 18 years old:
Copy of their current report card, letter from the school, or proof of current year registration record.
You can obtain all of these documents from your school.
Required documents for other family members: At times, we need to add other family members who live in your household to the application to determine eligibility. Because of this, you should plan to bring the following for other household members:
Other minor children in your family
Additional child’s Birth Certificate
Additional child’s Social Security number (just number, not actual card)
Proof of relationship to that minor child
Other Adults in your household (i.e., adult children, partner, spouse, other adult relatives)
If other adults works: 2 most recent pay stubs*, or W2 from 2023
Date of birth
Social Security number (just number, not actual card)
*DOCUMENTS MUST BE DATED WITHIN 60 DAYS OF YOUR SCHEDULED APPOINTMENT
If I am invited to an Orientation Appointment, what documents do I need?
All youth attending Orientation must bring:
Passport OR
Photo ID and Social Security Card or Birth Certificate
Y.O.U. must see these documents during Orientation, even though they were copied at Intake, because we must fill out the I-9 Employment Verification. We are legally required to view those documents when completing the I-9.
All documents must be unexpired.
If youth is 14 or 15, a Work Permit (Age and Schooling Certificate) is collected at Orientation.
A completed Work Permit Application is not acceptable. It must be the Work Permit.

Males who are 18 or 19 years old must also provide Selective Service Registration Verification.
If you do not bring all required documents to Orientation you will likely be rescheduled and you could forfeit your spot in the program.
Direct Deposit Information is also collected at Orientation if you do not want to receive your pay on a pay card.

What if I don’t know my social security number or don’t have a social security card?
If you are selected to participate you MUST have a social security card to continue with enrollment.
Getting a new social security card is free.
The website has instructions and the application to request a new social security card.
It takes more than 2 weeks to receive a new card (maybe longer due to the COVID-19 pandemic) once you submit the application.
If you need a new card, request one today.
I don’t have a certified copy of my birth certificate. How can I get one?
If you were born in Ohio, to get a certified copy of your birth certificate, contact the Cuyahoga County Bureau of Vital Statistics.

The cost for a certified copy of a birth certificate is $25.

What is a Work Permit and do I need one?
If you are 14 or 15 years old and you are offered a position in the Summer Jobs Program, you will need a Work Permit.
Work Permits are issued by the school district you are enrolled in and verify you are old enough to work and that you are in good standing to work since you are a minor and legally required to attend school. Employers, like Y.O.U., cannot issue Work Permits.
You must turn in a completed Application for Minor Work Permit to your school district and, at their discretion, they will issue an Age and Schooling Certificate.
The HOW TO GET A WORK PERMIT instruction manual and an Application for Minor Work Permit will be given to the PARENT/GUARDIAN (at their Intake Appointment) upon approval for the Summer Jobs Program.
Be advised that the Work Permit process can be lengthy and requires getting a physical exam at your family’s expense and having your physician sign off on their part of our WORK PERMIT application.
THE WORK PERMIT APPLICATION IS PRE-SIGNED BY Y.O.U as the Employer of record which is why it is not provided until after the Intake Appointment is completed.
Please do not use the blank work permit applications given by your child’s school or from online.
A completed Work Permit (Age and Schooling Certificate) must be turned in at Y.O.U. Orientation.
